Even though students are not in the school building in person at the moment, it is still critical that we have your most current contact information in case we need to reach you. This includes postal mail address(es), phone numbers (home, work, and cell, as applicable), and email address(es). 

• Emergency Care Form and Health Form (these must be reviewed by all families and updated if necessary - update online via Parent SIS/Blackboard 24-7))

2. Parent Consent Form - Digital Resources

Please visit our digital resources page to give consent for our use of a variety of programs. 

3. Student Rights & Responsibilities Booklet and Signature Page  

Parents and guardians, please review the Student Rights and Responsibilities and related resources. 

After reviewing the information, fill out the SR&R Parent Acknowledgement Form.

The form must be filled out one time per student.

4. Additional Registration/Demographics Forms

• Homeless Student Referral
• Annual Notice of Survey, Records, Curriculum, Privacy, and Related Rights and Opt-Out Forms

• Scoliosis Fact Sheet (for 5th and 6th graders)
• Student Health Insurance (optional)
